1) Identify Contractual/Fiscal Agent using the Contractual/Fiscal Agent Notification Form w/ Signature Page, Due April 30, 2007

Submit with the following documents:

  • IRS Determination Letter (if applicable)
  • Contractual/Fiscal Agent Organization Chart
  • Memorandum of Understanding
  • Minutes of the Community Mobilization Policy Board
2)      Submit the Six Year Strategic Plan - Due April15, 2007
3)      Attend an OSPI data system training.
4)      Complete a work plan on the Safe and Drug Free Schools and Community data system.  (Due April 30, 2007)
5)      Submit an annual budget.  (Directions Forecoming).
6)      Sign a contract (Directions Forecoming. Due June 30, 2007).
7)      Attend new contractor orientation (If new, Directions Forecoming).
